The records office is located in the front office of the school, across from the principal's secretary. The records office at Jesuit Dallas handles all student registrations, re-registrations, health forms, grades, and scheduling information for current students.
The records office also handles transcript requests for all alumni.

Notary Public Services are available in the Jesuit Records office and at the front desk at Jesuit from 9 a.m. to 3 p.m. each school day. If you require a notary during a different time, please call Diane Camara, x366, to schedule an appointment. State law requires all notarizations must be done in person by the person requesting the service along with proper id.

All graduates, withdrawal students and students in grade 9, 10, and 11 may request transcripts from the Records office. Students in grade 12 must request all transcripts from the College Coordinator in the Counseling office.
All requests for transcripts from the Records office will be completed within 48 hours. Official transcripts must be sent from Jesuit to the receiving party. All other requests will receive a student issued copy. All requests must be in writing. Please contact the Records office by fax, email or mail with your request including the address to which your transcript is to be sent.

Below, please find the Enrollment Forms necessary for incoming freshman and transfer students.
Health and Immunization Forms
Health forms are completed by each student entering the Freshman class and transfer students. The form lists the student's history of immunizations and current physical condition. This form is kept on file at Jesuit for a period of ten (10) years after graduation. Texas state law requires that all students attending school must have current immunizations. Incomplete forms will be returned to the student. A report is generated each month to determine which students are not in compliance with the immunization requirements. Letters are then sent to the parent notifying them of the time they have to update a student's immunization record. Students who do not have Health foms on file by the end of the first day of school or do not return proof of immunizations by due dates will be asked to remain out of school until the verification of immunization is received. All students must turn in the original form mailed in their acceptance packet. Medical Release Forms
Medical Release forms are sent to each student entering the Freshman class and Transfer students. Should a student require medical treatment while in the care of a Jesuit faculty or staff member, the form is readily available to verify the parents' permission to seek treatment. The form lists the student's last tetanus shot, physician's name and physician's address. The form must be signed by the parent and notarized. This form is kept on file at Jesuit until the student graduates. The form allows the student to participate in off-campus activities. Forms are due to the Records office with registration. Students who do not have Medical Release forms on file by the end of the first day of school will be asked to remain out of school until the completed document is received. All students must turn in the original pink document mailed to them in their acceptance packet.
Students request a Verification of Enrollment through Polanco, which they access from an on-campus computer, except during the summer when these forms are available at the front desk.
Verification of Enrollment (VOE)
Students needing a VOE/TEA for the purposes of obtaining or renewing a Texas drivers license during the school year may request forms from the front desk at Jesuit between the hours of 9 a.m. and 3 p.m. During summer months, students may request forms from the front desk at Jesuit between the hours of 9 a.m. and 1 p.m. Students must present some form of picture ID to obtain the form. Proof of Enrollment forms are required by the Texas Department of Motor Vehicles until the student reaches the age of 18 years.
Incoming Freshman must contact their previous school to verify their last 90 days of attendance.
